I am authorised to write to confirm as follows:

I consent to your obtaining, storing and processing information about me (which may be obtained from any source) and I am aware that this information will be used by you for the purposes of credit or financial assessments, the prevention of fraud and money laundering or training. This information may also be used to assist you in determining whether to continue the Agreement and the terms upon which the Agreement will be continued and to allow us to store your timesheets, process them and pay your invoices on a weekly basis.

I consent to your passing any information to 1PS (“1st PS Limited”) for the purposes of providing their services to the Agency which include but are not limited to Contract Management, Time Sheet, Expense & Invoice Management, Funding and making Payments to me on behalf of the Agency.

I understand that if I wish to have a copy of any information held by you about me, or details of any other parties from or to whom data may be passed then I should contact the Data Protection Officer, I understand that if I require a copy of any information held by you, I should make a Subject Access Request (SAR) in writing to the agency.

I understand that if I wish to withdraw consent to the Agency accessing, storing and processing my personal data, I should raise a Request to Withdraw Consent to the Data Protection Officer.
